Despite the best of technologies and the acumen designs of aircraft manufacturers, we are still left in the dark. In this article, we explore the role of investigating agencies in determining the cause of these crashes. We delve into the challenges they face, including resource constraints, time factors, and national or political considerations that can compromise the integrity of the probe. In both our personal and professional lives, we occasionally encounter situations that leave us speechless, where all logic seems to fail, and we face confusion or frustration. Consider the incidents involving flights MH 370, AF 447, JAL 123, TWA 800, Egypt Air 990, and Adam Air 574: some crashed, others vanished without a trace, leaving no debris or distress signals. Questions arise about what happened to these aircraft and their passengers, whether they're alive or not, prompting a loss for words and wonder if there's a higher power at play.
Certain situations are out of our control and yet they're hard to dismiss, especially for those in the flying crew who might ponder on them more often than non-crew members. These incidents can occur unexpectedly at any place and time. So, it raises the question: Why hasn't there been any initiative towards corrective measures? With advanced technology and sophisticated aircraft designs

available, why do we still face these unresolved issues?
Should we accept in all humility that we as human beings have limited resources, knowledge, capabilities and, therefore if situation arises like a fatal crash, then in the end we accept that it had to happen and has happened. Period.
It's important to recognize our human limitations in resources, knowledge, and abilities, and thus, when a tragic event such as a fatal crash occurs, we may have to come to terms with it as an inevitable outcome. That's the reality.
In most of the cases, the easiest was to point fault is with the pilot “Pilot Error”, he/ she is dead cannot defend themselves and very cool for the airlines and aircraft manufacturers. Win -Win situation, the crash investigation unresolved! The deteriorating weather, the pilot error are the figments where nobody is alive, so no one can be prosecuted, and the file is kept in the cold storage. Khalaas !
When events unfold, our minds swiftly seek to grasp the unfolding tragedies, often revealing design flaws or maintenance issues. The human mind excels in interpreting the world, swiftly crafting explanations and understanding from even the smallest hints. what is required is intention to go to the root level of disaster, and then would know, whether it is man-made or design fault or situation beyond the control.
Humans do not make mistakes constantly. Ensure things are visible for us to learn from in the future. And that is the call of the investigating agencies.
Coming to more humanistic approach-
Has anyone thought of the family members of the people whose relatives went missing? For news agencies it is the number which matters, 280 passengers died or 340 injured, so on and so forth. That many women, so many men and children, infants etc. In the after math of the balancing the equation as total number of the diseased should be equal to the men +women + children +infants +crew members on board the aircraft. Do we have some kind of conscience where we go beyond the available sources to find out the exact cause of crash? And also, to mitigate the effects of the incident / event in terms of agonising pain of the individuals and some kind of assurances for the safety perspective to be enforced, so that similar incidences/ events could be averted. Or, if they do happen as we have acceded at some point that things go beyond control, we have certain measures to pacify the wounds of the effected people.
Here comes the role of the investigating agencies.
The investigating agencies may prioritize certain findings or outcomes, potentially ignoring evidence.
The consciousness and the mindset of the investigating agencies can play a significant role in crash probes. Pre- conceived ideas or probes they could have been involved could have an effect on the result findings mechanism/machine. Of course, we know the investigating agencies should be impartial, transparent with open communication. Multidisciplinary expertise and collaboration coupled with evidence-based analysis can provide effective findings in the crash probes. Clear and unbiased reporting with the vision of further enhancement can take us to a long way.
We can also not ignore resource constraints, time factor. Investigations can also be influenced by national or political considerations, thereby compromising the integrity of the probe. Communication and collaboration by different stake holders can definitely exploit the outcome of the findings. International partners can hinder investigations progress and effectiveness. We as human beings fail to understand from the past mistakes or investigations short comings which can Of course, we know the investigating agencies should be impartial, transparent with open communication. In summary, it can be stated that while investigating agencies cannot escape accountability, there lacks a system to identify and address significant issues that may arise unexpectedly.
